Ukončení 1. linie anti-HER2 terapie u metastazujícího karcinomu prsu při dosažení výjimečné odpovědi

SOUHRN

Zavedení terapie proti receptoru 2 pro lidský epidermální růstový faktor (human epidermal growth factor receptor 2, HER2) do klinické praxe u pacientek s metastazujícím HER2 pozitivním karcinomem prsu zásadně změnilo prognózu tohoto onemocnění. Na základě výsledků studií bylo zjištěno, že nezanedbatelná část pacientek je schopna dosáhnout dlouhodobě trvající odpovědi při první linii anti-HER2 léčby. Většina z nich podstupuje nadále anti-HER2 léčbu na základě doporučení odborných společností. Existuje předpoklad, že léčba může být přerušena při trvání kompletní remise onemocnění u tzv. dlouhodobých respondérek. Doposud nebyly identifikovány jednoznačné klinické, biochemické nebo molekulárně genetické aspekty této skupiny. Momentálně probíhající studie nám mohou přinést poznatky, které budou klinicky relevantní při rozhodování o ukončení anti-HER2 terapie u pacientek s dlouhodobou odpovědí na anti-HER2 léčbu.

SUMMARY

The introduction of anti-human epidermal growth factor receptor 2 (anti-HER2) therapy into clinical practice in patients with metastatic HER2-positive breast cancer has fundamentally changed the prognosis of this disease. Based on the results of the studies, it was found that a non-negLigibLe part of the patients is able to achieve a Long-Lasting response in the 1^st Line of anti-HER2 treatment. Most of them continue to undergo anti-HER2 treatment based on the recommendations of professional societies. There is an assumption that the treatment can be interrupted if a complete remission of the disease persists in so-caLLed Long-term responders. Clinical, biochemical or moLecuLar-genetic aspects of this group have not yet been cLearLy identified. The currentLy ongoing studies may bring us insights that wiLL be cLinicaLLy reLevant when deciding whether to discontinue anti-HER2 therapy in patients with a Long-term response to anti-HER2 treatment.
