Postavení imunoterapie v léčbě nádorů čípku a těla dělohy

SOUHRN

Chemoterapie na bázi platiny, taxanu a eventuálně antracyklinů byla dlouhou dobu základem léčby pacientek s lokálně pokročilými a metastazujícími nádory čípku a těla dělohy, u nichž byly vyčerpány možnosti chirurgické léčby a radioterapie. První molekula, která získala registraci v případě léčby čípku dělohy, byl bevacizumab v kombinaci s chemoterapií a nyní se nově do léčebných algoritmů dostala také imunoterapie. Imunoterapie pembrolizumabem je v tuto chvíli indikována v kombinaci s chemoterapií a eventuálně bevacizumabem v první linii paliativní léčby pacientek s karcinomem čípku dělohy a v druhé a vyšších liniích je možno indikovat pembrolizumab nebo cemiplimab. Podobně v případě nádorů těla dělohy je možné na základě proběhlých studií podávat u předléčených pacientek pembrolizumab pro léčbu nádorů s defektem v mismatch repair genech / vysokou mikrosatelitovou instabilitou (dMMR/MSI-H), eventuálně bez ohledu na status MMR kombinaci pembrolizumabu s lenvatinibem. Imunoterapie tak představuje nový standard léčby těchto nádorů.

SUMMARY

Platinum, taxane and eventually anthracyclines-based chemotherapy represented gold standard for the treatment of the metastatic cervical or endometrial cancer, where surgery or radiotherapy didn't represent the treatment of choice. As first targeted molecule which gained the approval was bevacizumab in combination with chemotherapy and recently immunotherapy has gained the approval as well. Immunotherapy with pembrolizumab is indicated in combination with chemotherapy and eventually with bevacizumab in the first line of the palliative treatment of the cervical cancer as well as in the second and higher lines pembrolizumab monotherapy or cemiplimab. Similarly in the case of endometrial cancer according to the recent clinical trials pembrolizumab may be used in the case of tumors with mismatch repair deficiency / high microsatellite instability (dMMR/MSI-H) or without the mutated MMR status the combination of pembrolizumab with lenvatinib is the treatment of choice. Immunotherapy represents the new treatment standard in the treatment of the cervical or endometrial cancer.
