Novinky v systémové léčbě karcinomu endometria

SOUHRN

Systémová terapie je základním pilířem léčebného přístupu u pacientek s pokročilým nebo recidivující m endometriálním karcinomem. Dosavadní možnosti systémové léčby byly limitovány na použití chemoterapie a hormonoterapie, navíc bez definovaného standardního postupu pro druhou a vyšší linii léčby. Změnu v chápání endometriálního karcinomu jako heterogenní skupiny onemocnění s odlišným biologickým chováním i v rámci téhož histologického typu přinesly výsledky programu The Cancer Genome Atlas (TCGA) s následným definováním čtyř molekulárních subtypů karcinomu endometria s odlišnou prognózou. Molekulární testování umožnilo také rozvoj dalších alternativ systémové léčby endometriálního karcinomu, tedy využití imunoterapie, její kombinace s chemoterapií, léků cílících na inhibici angiogeneze i dalších možností molekulárně cílené léčby.

SUMMARY

Systemic therapy remains to be the cornerstone of treatment for patients with advanced/recurrent endometrial cancer. Possibilities of current treatment have been limited to chemotherapy and endocrine therapy, without standard of care for next line therapy. The change in understanding of endometrial cancer as a heterogenous disease with different biological behavior even within the same histologircal type was brought by results of The Cancer Genome Atlas (TCGA) analyses with definition of four molecular subtypes of endometrial cancer with different prognosis. Molecular testing has brought new alternatives for systemic treatment of endometrial cancer such as immunotherapy, combination of immunotherapy + chemotherapy, anti-angiogenesis therapy and other molecular targeted therapy.
