Novinky v radioterapii nádoru prsu

SOUHRN

Radioterapie je stále nezbytnou součástí komplexní léčby karcinomu prsu, přestože pokroky v diagnostice, chirurgii a systémové léčbě zásadně změnily strategii léčby karcinomu prsu. Mezi aktuální témata v léčbě zářením u časného karcinomu prsu patří zkrácení délky léčby, identifikace skupiny pacientek, které nepotřebují adjuvantní radioterapii a snižování rizika pozdních poradiačních změn s použitím nových technik radioterapie. U lokálně pokročilého karcinomu prsu po mastektomii se diskutuje o skupině pacientek, u nichž je nutné provedení radioterapie hrudní stěny a regionálních lymfatických uzlin. Aktuální je i načasování radioterapie a rekonstrukce prsu. Terapie zaměřená na metastázy pomocí ablativní radioterapie je novou oblastí zájmu i u karcinomu prsu, včetně její role a dopadu u oligometastatického karcinomu prsu.

SUMMARY

Radiotherapy is still an essential part of comprehensive breast cancer treatment, although advances in diagnostics, surgery and systemic therapy have fundamentally changed the strategy of breast cancer treatment. Current issues in radiation therapy for early breast cancer include shortening the duration of treatment, identifying a group of patients who do not need adjuvant radiotherapy, and reducing the risk of late consultative changes with the use of new radiotherapy techniques. In locally advanced breast cancer after mastectomy, there is a debate about the group of patients who need radiotherapy of the chest wall and regional lymph nodes. The timing of radiotherapy and breast reconstruction is also an issue. Metastasis-targeted therapy with ablative radiotherapy is also a new area of interest in breast cancer, including its role and impact in oligometastatic breast cancer.
