SOUHRN

Neoadjuvantní chemoradioterapie následovaná chirurgickou resekcí představuje zlatý standard léčby nádorů jícnu a gastroezofageální junkce. Postavení neoadjuvantní chemoradioterapie je definováno několika studiemi a její indikace je poměrně jasná, kdežto vlastní indikace chirurgického výkonu, ačkoli v České republice obvyklá, je mnohem méně jednoznačná. Benefit adjuvantní chemoterapie byl prokázán v několika retrospektivních hodnoceních a v současné době máme i data prokazující benefit imunoterapie ve formě checkpoint inhibitorů po komplexní léčbě nádorů jícnu a gastroezofageální junkce u pacientů, kteří nedosáhli kompletní remise po neoadjuvantní chemoradioterapii, a to na základě klinické studie CheckMate 577. Nivolumab získal registraci v této indikaci, nicméně úhrada z veřejného zdravotního pojištění v tuto chvíli chybí.

SUMMARY

Neoadjuvant chemoradiotherapy followed by surgical resection represents a gold standard of the treatment of the esophageal and gastroesophageal cancer. The position of the neoadjuvant chemoradiotherapy in the treatment algorithm is clear however the indication of the esophagectomy is quite vague. Several retrospective studies have confirmed the benefit of the adjuvant chemotherapy and at this moment there are studies which showed the benefit of adjuvant immunotherapy as check-point inhibitors in patients with esophageal and gastroesophageal cancer which didn't achieve complete remission after neoadjuvant chemoradiotherapy based on the CheckMate 577 study. Nivolumab gained the registration in this indication in Czech Republic however the healthcare reimbursement is still missing.
