Nové možnosti v léčbě nádorů žlučníku a žlučových cest

SOUHRN

Chirurgická léčba představuje bohužel jedinou léčebnou modalitu, která poskytuje pacientům s nádory žlučníku / žlučových cest šanci na dlouhodobé přežívání. Standardem léčby pacientů s lokálně pokročilým či metastazujícím onemocněním je v tuto chvíli kombinace ciplatiny s gemcitabinem, když většina cílené léčby bohužel u tohoto typu nádoru nepřinesla očekávané výsledky. V dalších liniích léčby jsou obvykle indikovány kombinace s oxaliplatinou nebo taxanem. Durvalumab je první molekulou ze skupiny imunoterapie, která prokázala účinnost u nádorů žlučníku / žlučových cest a získala registraci pro tuto indikaci na základě klinické studie TOPAZ-1. Chemoterapie v kombinaci s durvalumabem splnila primární cíl studie, když došlo ke statisticky signifikantnímu prodloužení mediánu celkového přežití léčených pacientů. Durvalumab představuje novou léčebnou možnost u pacientů s nádory žlučníku / žlučových cest, kterou je vhodné zvážit v první linii paliativní léčby.

SUMMARY

Surgery represents unfortunately only treatment option of the patients with gallbladder / biliary tract tumors, which provides the chance for long term survival. The gold standard of the treatment of the locally advanced or metastatic disease is the combination of cisplatin with gemcitabine, when targeted therapy didn't provide expected results. In further lines of the palliative treatment with chemotherapy combinations with oxaliplatin or taxane are usually indicated. Durvalumab is the first molecule from the group of immunotherapy, which showed effectivity in the treatment of the gallbladder / biliary tract cancer and gained registration in this indication based on the clinical trial TOPAZ-1. Chemotherapy with durvalumab fulfilled the primary endpoint of the trial, when median overall survival of the patients was improved with statistical significance. Durvalumab represents new treatment option for the patients with gallbladder / biliary tract cancer and should be considered in the first line of the palliative treatment.
