Hormonální léčba versus chemoterapie u pacientek s agresivní formou karcinomu prsu – klinická studie RIGHT Choice

SOUHRN

Paliativní léčba hormonálně dependentních nádorů prsu je v současnosti založena na hormonální léčbě v kombinaci s inhibitorem cyklin dependentní kinázy 4/6 (cyciin-dependent kinase 4/6, CDK4/6). Tato kombinace je podložena dostatečnými daty z klinických studií a je zlatým standardem léčby. Výjimku tvoří skupina pacientek s velmi rychle probíhajícím nádorovým onemocněním, eventuálně viscerální krizí, kdy chemoterapie, ať již v monoterapii, nebo v kombinaci představuje stále preferovaný postup. Změnu z tohoto zažitého algoritmu přinesla až klinická studie RIGHT Choice, která hodnotila kombinaci ribociclibu s hormonální léčbou versus kombinovaná chemoterapie právě u této skupiny pacientek s velmi špatnou prognózou. Výsledky studie potvrdily benefit hormonální léčby v kombinaci s inhibitorem CDK4/6 ve všech studovaných parametrech, a to včetně kvality života léčených pacientek, a je podkladem změny standardu léčby.

SUMMARY

The palliative treatment of the hormonal dependent breast cancer patients is based on the hormonal treatment together with CDK4/6 inhibitors. This combination is the gold standard according to performed clinical trials. One exception is the group of patients with rapidly progressive disease or patients with visceral crisis, where chemotherapy as monotherapy or combination still represents preferred treatment algorithm. The change from this treatment brought the clinical trial RIGHT Choice. This was the trial which compared the treatment with the combination of hormonal treatment together with CDK4/6 inhibitors and chemotherapy combination in this group of patients with very poor prognosis. The results of the trial clearly demonstrated the benefit of the hormonal treatment together with CDK4/6 inhibitors in all studied parameters including the quality of life. The clinical trial RIGHT Choice represents the background for the change of the treatment standard of this group of patients.
