The role of radiotherapy in the treatment of gastroesophageal junction adenocarcinoma

The treatment of locally and regionally advanced gastroesophageal junction adenocarcinomas is a very controversial topic. The therapeutic approach may include preoperative chemoradiotherapy, which is commonly used in esophageal cancer, as well as perioperative chemotherapy, eventually postoperative chemoradiotherapy, which are treatment alternatives used in gastric cancer. Due to the non-uniform approach in the treatment of gastroesophageal junction tumors, the benefit of radiotherapy is not clearly defined in these tumors, although it seems that the addition of radiation may be associated with some benefit over chemotherapy alone.

Mechanism of synergy of radiotherapy and immune checkpoint inhibitor therapy

Modern immunotherapy using immune check-point inhibitors has experienced a huge boom in routine clinical practice. In preclinical and clinical practice, synergy of immunotherapy and radiotherapy is observed. The reason is the ability of radiotherapy to activate anti-tumor immunity, even outside the irradiated tumor (abscopal effect). Activated anti-tumor immunity is one of the important conditions for the therapeutic effect of immune check-point inhibitors.

4D MR adaptive brachytherapy of uterine cervix carcinoma in FN Hradec Králové

Radiotherapy and particulary brachytherapy have been major treatment modalities for cervical cancer for over 100 years. Most of this time the brachytherapy treatment planning was based on radiographs and reference point dosimetry. Due to research of European Society for Radiotherapy and Oncology (GEC-ESTRO) a 3D brachytherapy planning, based on the magnetic resonance imaging with adaptation of the treatment to actual tumor regression in time (4D), has become a standard of care in the last decade. University hospital in Hradec Kralove is the first center in the Czech Republic to implement the adaptive brachytherapy planning of cervical carcinoma based solely on the magnetic resonance imaging for each fraction.

Current approaches to head and neck cancer treatment

The present review summarizes current treatment approaches for head and neck tumors. The basic therapeutic modalities for squamous cell carcinomas are surgery and radiotherapy. Systemic treatment is a part of concurrent chemoradiotherapy or it is indicated with a palliative intent alone. A new approach to palliative treatment is the use of immunotherapy. A partly different approach compared to other head and neck tumors is taken into consideration in nasopharyngeal cancer and salivary gland tumors.
