Benefit of panitumumab in colorectal carcinoma treatment – case report

Epidermal growth factor receptor (EGFR) inhibitors have become a standard part of combined treatment regimens for metastatic/advanced colorectal cancer. Their long-term use is linked to the mutational status of KRAS and NRAS genes, furthermore it is recommended rather for left-sided tumors.1,2 Following case report shows the significant benefit of EGFR inhibition with panitumumab during the treatment of a patient with a locally advanced rectosigmoid tumor, which, even after years, eventually led for a high hope for complete recovery.

Trifluridine and tipiracil for the treatment of advanced colorectal cancer – case report

The treatment of colorectal cancer in the first two lines is based on a combination of chemotherapy (FOLFOX and FOLFIRI, also in variations) and biological therapy in the form of vascular endothelial growth factor (VEGF) and epidermal growth factor (EGFR) inhibitors. Although this therapy is challenging and brings a lot of side effects, many patients are in good condition after progression. Especially for these patients the treatment with trifluridine/tipiracil is appropriate. This preparation contains a nucleoside analogue trifluridine and a thymidine phosporylase inhibitor tipiracil - inhibitor of the enzyme metabolizing just that nucleoside analogue. Trifluridine/tipiracil demonstrated efficacy in phase 3 clinical trial RECOURSE.1 Based on that the treatment was approved as well in the Czech Republic.

Therapeutic significance of microsatelite instability in colorectal cancer

Colorectal carcinoma is heterogenous disease. Methods of molecular testing reveals new alterations, often without better understanding. However, some of these molecular tests, such as KRAS/NRAS or BRAF mutations, are already projected into daily practice. Microsatellite instability represents one of these detectable changes. Nor it helps us to decide on adjuvant chemotherapy, but it is increasingly important with the advent of drugs that affect the immune system.
